Workers’ Compensation Injuries

On-the-job injuries are becoming more and more common in our society. From traditional traumatic injuries to repetitive injuries such as Carpal Tunnel Syndrome, the work place can be full of perils that may result in injury to your while at work. If you suffer an injury on the job, you have the right to open a Workers’ Compensation claim as result. Under Oregon law, you can receive 18 chiropractic visits or sixty days of care if you see your chiropractor as your “attending physician” first after an injury. If you have been injured at work, please contact us to set up an appointment as soon as possible to get your injury treated, resolved, and get you back to great health and back to work!

If you are already seeking care from another doctor (the “attending physician”), you can get a referral from that doctor for chiropractic care or change your attending physician to your chiropractor. Patients can change their attending physician up to two times after the initial choice. In the state of Oregon, treating physicians must qualify with the state’s Workers’ Compensation Board to treat patients under this system. Dr. Seth Alley, DC, CKTP and the Sylvan Chiropractic Clinic and Wellness Center are fully qualified with the state’s offices to provide services to injured workers.

If you have been injured on the job and are seeking care and treatment at our clinic, please complete the Workers’ Compensation Patient Form in addition to the Chiropractic Patient Registration Packet before your first visit so that we can correctly handle your case and provide you with the best care available.
